Incident Reports
All incidents, near misses, injuries, and security events must be reported promptly. Use the button below to submit a report directly to the PPS document system.
If there is an immediate threat to life or safety, call Triple Zero (000) before completing any report. Officer safety is the priority. Reports must be submitted within 24 hours of any incident.
How to Submit a Report
Follow these four steps every time you lodge an incident report.
Note the date, time, location, people involved, and a clear description of what occurred.
Click 'Submit Incident Report' below to open the report form in SharePoint. Fill in all required fields.
Upload any photos, CCTV references, or supporting documents directly to the SharePoint folder.
After submitting, inform your shift supervisor or the Admin team that a report has been lodged.
Types of Reportable Incidents
The following must always be formally reported.
Theft, assault, trespass, threatening behaviour, or breach of security protocols.
An event that did not cause injury or loss but had the potential to do so.
Any work-related injury, illness, or medical event involving staff or a member of the public.
Damage to client property, PPS equipment, or vehicles during an engagement.
Any incident requiring police attendance or resulting in an arrest or statement.
Formal complaints from clients, staff, or members of the public.
What to Include
Every report must cover these points.
- Full name and licence number of reporting officer
- Date, time, and exact location of the incident
- Names and contact details of any witnesses
- Clear, factual description of what occurred
- Actions taken at the time
- Whether police or emergency services were called
- Any injuries sustained and first aid provided
- Client or site name
